PAY GRADE: $17.47  EEOC CODE: Professional FLSA STATUS: Non-Exempt

SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm

The mission of Gulf Bend Center (GBC) is to improve the quality of life in our community for individuals and their families by providing excellent and trusted care for wellness. GBC is committed to providing quality person-centered, trauma-informed care to individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.

LEVEL OF SUPERVISION REQUIRED: General

JOB PURPOSE:

The Community Resource Center (CRC) Navigator will serve as a liaison who works directly with individuals to connect them with community resources and participates in developing community support systems and networks. Responsible for connecting to and collaborating with community partners, federal and state agency services, and other local nonprofits to incorporate their services into the CRC. This position will participate in local advisory committees, implement an intake process to include regular data collection and analysis, and organize and facilitate (as needed) community conversations, Community Health Worker (CHW) meetings, focus groups, strategy sessions, and presentations. They will also be responsible for maintaining a community resource manual.

ESSENTIAL FUNCTIONS and DUTIES:

  1. Establish outreach strategies and referral pathways for vulnerable populations such as minority populations and individuals residing in economically disadvantaged communities. Manage intake, assistance, and follow-up with cases.
  2. Maintain relationships with service providers and work closely with other area navigators.
  3. Connect community members with services needed, including assessing individual barriers to determine relevant referrals.
  4. Collect and enter participant data, manage scheduling, and track referrals within data management software.
  5. Continuously update community resource manual.
  6. Maintain accurate logs, contact notes and files for CRC participants.
  7. Serve as point of contact for mobile CRC locations.
  8. Attend and participate in staff meetings, community health worker meetings, and community collaboratives to develop and maintain working connections with area resources, including other area navigators.
  9. Uses the trauma-informed approach when providing services and working with others and understands trauma-informed services.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY:

Trauma-informed: A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six fundamental principles of a trauma-informed approach include safety, trustworthiness, and transparency; peer support; collaboration and mutuality; empowerment, voice, and choice; and cultural, historical and gender issues.

  1. Understanding of clinic operations related to customer registration and referrals.
  2. Ability to meet deadlines and goals, strong time management, detailed oriented, passionate about providing quality customer service, and ability to link customers to resources within Gulf Bend Center and the surrounding counties.
  3. Must communicate effectively both orally and written.
  4. Strong decision making and organizational skills.
  5. Ability to operate a computer, office equipment and office software to include Microsoft Word, Excel, Outlook, Teams, Zoom. Ability to type 40 wpm.
  6. Must have good organizational skills to achieve multiple deadlines.
  7. Exhibits cooperative teamwork with other staff to accomplish the efficient operation of the program.
  8. Models professionalism by appropriate dress, language, ethics and work habits.

EQUIPMENT, TOOLS AND MATERIALS

  1. Computers, telephones, projectors, calculators, fax machine, and copy machine are used in performance of essential job duties.
  2. Various forms of paperwork are handled in performance of essential job duties.

EDUCATION AND EXPERIENCE REQUIRED:

Required: Bachelor's degree in social work, Psychology, Human Services, Public Health, or a related field from an accredited college or university.

Preferred: Certification as Community Health Worker through the Texas Department of State Health Services and two (2) years' work experience with underserved populations.

DEGREE AND/OR CERTIFICATION REQUIRED:

  1. CHW certification through DSHS; can be obtained simultaneously to employment in role as navigator within initial year of employment.

OTHER REQUIREMENTS OR CONDITIONS:

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  3. Good driving record as defined by Gulf Bend Center.
  4. Must complete all training requirements prior to the due date. Attend required training and consortia meetings. Relias training and any other training required for the role is 100% compliant.
  5. Must have computer experience to include Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  6. Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
