LEVEL OF SUPERVISION REQUIRED: General
JOB PURPOSE:
Provides psychosocial rehab services, skills training services, and routine case management services to adults with severe and persistent mental illness.
ESSENTIAL FUNCTIONS and DUTIES:
2. Develops a Person-Centered Recovery Plan in collaboration with the customer, that identifies the appropriate goals and objectives, and the frequency and duration of services to be provided based upon the TRR assessment according to TAC code and Center policy.
3. Provides authorized services (psychosocial rehab services, skills training services, and routine case management) to consumers according to the Recovery Plan using independent clinical judgment to assess client needs in accordance with Medicaid Rule, HHSC Contract, and UM guidelines.
4. Screens for risk of suicide, completes Safety Plans, and provides services to individuals on the Suicide Safe Care Pathway as needed.
5. Provides ninety-five percent (95%) of services in the consumers' natural environment such as home or community.
6. Meets productivity standard by delivering authorized services in accordance with agency guidelines.
7. Submits all documentation in electronic healthcare record (EHR) including forms, services, disclosures, and telephone contacts etc. within two (2) business days.
8. Monitors and manages caseload to prevent expiration of TRR Assessment, consent forms, financial assessments, and diagnosis to prevent lapses in services.
9. Attends and actively participates in all team meetings and meets with supervisor to enhance professional growth, review and improve case management services, and address relevant administrative matters.
10. Meets with individuals assigned to their caseload within 7 days to provide services for continuity of care.

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral, or human services or a minor in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling, sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: At least six months' experience in providing mental health services to adults, children and adolescents preferred.

1. BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY:
1. Program specific: Working knowledge of the nature of serious mental illness and related treatment modalities, interventions and techniques; of different types of assessments and their uses in recovery planning; of consumers' rights; of local community resources and service delivery systems such as housing, social, welfare, educational needs of client record documentation requirements; and of client services plan development and implementation. Demonstrated ability to interview and assess clients, using appropriate assessment tools, and observe, record and report on an individual's functioning. Ability to read and understand assessments, evaluations, observation, and use in developing treatment plan. Ability to identify community resources and services for clients and coordinate provision of services.
2. Communication: Strong written and verbal communication skills to communicate clearly and effectively with strong organizational skills and the ability to interface effectively with all levels of the organization. Maintains confidentiality.
3. Task management: Ability to perform tasks effectively and efficiently and to handle multiple priorities in a confidential environment. Must have strong attention to detail. Ability to plan and schedule work and implement directives without constant supervision. Ability to establish and maintain effective working relationships with staff, other agencies and the public using tact, courtesy, and good judgment.
7. 10. Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.

2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
