JOB PURPOSE:
Provides case management, skills training, and medication training to children/adolescents with a serious emotional disturbance and services to the primary caregiver or LAR, in the school, community and/or clinic.
EDUCATION AND EXPERIENCE REQUIRED:
Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling, sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: At least six months' experience in providing mental health services to adults, children and adolescents.
DEGREE AND/OR CERTIFICATION REQUIRED:
1. BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

LEVEL OF SUPERVISION REQUIRED: General

GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.
JOB PURPOSE:
Provides case management, skills training, and medication training to children/adolescents with a serious emotional disturbance and services to the primary caregiver or LAR, in the school, community and/or clinic.
ESSENTIAL FUNCTIONS and DUTIES:
1. Completes TRR assessment in accordance with Center policy and HHSC. Initial TRR assessments are completed within 7 days of assignment and must be updated at least two weeks prior to expiration for continuity of care.
2. Formulates a Recovery Plan to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the TRR assessment according to TAC code, HHSC Contract, and Center policy.
3. Provides authorized Case Management services to consumers and their supports according to the Recovery Plan developed using independent clinical judgment in accordance with Medicaid Rule, HHSC Contract, and UM Guidelines.
4. Provides ninety-five percent (95%) of services in the consumers' natural environment (including at home, school, and community).
5. Submits treatment recommendations through Utilization Review based on clinical necessity per supervisor review.
6. Submits all documentation in electronic healthcare record (EHR) including forms, services, disclosures, and telephone contacts etc within two (2) business days.
7. Meets productivity standard.
8. Uses assigned curriculums effectively and according to fidelity when providing skills training.
8. Tracks and manages caseload to avoid expiration of Consumer's required documents and forms to avoid lapse in services.
9. Actively participates in all team meetings and supervisions as a means of enhancing professional growth, reviewing and processing the provision of case management services, and appropriately addressing concerns and other requests as needed.

12. Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
