Case Manager, Community Response Team (CRT)

JOB TITLE: Community Response Team (CRT) Case Manager

SHIFT: Full time; monthly rotating shifts Monday through Thursday or Tuesday through Friday, 8:00am to 7:00pm, some evenings

JOB PURPOSE:

Works as member of the Community Response Team (CRT), serving as an advocate for people throughout our GBC Region and coordinate programs of support throughout the community for the benefit of people who have mental health needs. Provides crisis intervention services as needed to reduce symptoms with the intent to prevent admission to a more restrictive environment and case management services to link to appropriate community resources and services to divert from incarceration and hospitalization. Works in cooperation with family members and community organizations including, but not limited to, law enforcement, hospital staff and other community providers. Services are provided within Gulf Bend Center's 7-county service area.

EDUCATION AND EXPERIENCE:

Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred: Work experience in mental health and/or experience working within law enforcement setting preferred.

DEGREE AND/OR CERTIFICATION REQUIRED:

BA/BS in field specified above. 

ESSENTIAL FUNCTIONS and DUTIES:

  1. Co-responds with law enforcement to calls and/or referrals from family/friends, community partners, or other GBC staff.
  2. Responds to all calls from police dispatch appropriately within one hour of activation by the requesting county.
  3. Provides case management services by gathering information, identifying needs, and evaluating safety.
  4. Links customers and/or their families to services and community resources to meet their needs.
  5. Provides crisis intervention services as needed per HHSC contract, Medicaid Rule, and Center policy.
  6. Completes all paperwork according to Medicaid Rule, Grant Contract and Center Policy.
  7. Assists with and/or completes emergency warrants as necessary.
  8. Maintains a caseload, including enrolling and discharging customers under the Diversion program.
  9. Completes C-SSRS and Safety Plan when needed to identify and address safety concerns.
  10. Meets job expectations

  1. Program specific: General knowledge of community resources, and how to access them. Strong decision-making, ability to problem solve, and exhibit sound judgment under stressful situations.

  6.   9. Fleet: Completes fleet checklist as required. Report all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance
  10. Task management: Ability to perform tasks effectively and efficiently and to track and meet multiple deadlines in a confidential environment. Must have strong attention to detail. OTHER REQUIREMENTS OR CONDITIONS:

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
  3. Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Microsoft Edge, Adobe Acrobat, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax).
  4. Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required by community partners that is required for the role is 100% compliant.
  5. Must maintain regular attendance and reliability 100% of the time to ensure consistency and completeness of program's processes. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 7:00pm, or agreed-upon hours. Evening and weekend work may be required as job duties demand.
