JOB TITLE: Diversion Continuity of Care (COC-PESC) Case Manager
WORKSITE LOCATION: 1409 LaValliere Street, Victoria, TX 77901; outlying jails within 7 county catchment area
STARTING PAY: $18.47 EEOC CODE: Professional FLSA STATUS: Non-Exempt
SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm
EDUCATION AND EXPERIENCE REQUIRED:
Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: Work experience in mental health and/or experience working within jail setting.
DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE):
BA/BS or higher in field specified above.
ESSENTIAL FUNCTIONS and DUTIES:
- Responds to referrals (to include jail notifications, jail request forms, CCQs, and referrals from GBC staff, jail staff, and family/friends) from the six surrounding county jails appropriately within 2 business days.
- Assesses individuals to identify current needs and follows up to coordinate and link to community supports and services to meet these needs.
- Completes crisis assessments as needed, to be activated by jail staff or MCOT, or if need arises during routine COC visit.
- Documents daily in spreadsheet to track the number of inmates and outcome of visit.
- Effectively communicates with GBC staff on all current GBC clients that are receiving COC services in jail.
- Provides follow up in the community when individuals are released from jail.
- Coordinates with jail staff for visits with individuals and release date.
- Meets with community outreach providers to build relationships and educate on COC services.
- Responds to court ordered assessments by completing crisis assessment with supporting documentation and returning to magistrate within timeframe as needed.
- Participates in all required team meetings as scheduled by supervisor unless excused by supervisor.

KNOWLEDGE, SKILLS, ABILITIES, AND COMPETENCY:
- Program specific: Responsible for completing screenings, assessments, effective coordination of appropriate community services and linking to available community resources. Works closely with the criminal justice system and acts as liaison to work in cooperation with community organizations including, but not limited to, law enforcement and other community providers. May require travel within the 7 counties. Meets multiple deadlines and reporting requirements. Work is performed under the general supervision of the Team Lead for Community Response Team with considerable latitude for the exercise of discretion and independent judgment. Possess a general knowledge of community resources, and how to access them.
- Communication: Strong verbal and written communication skills with legible handwriting to communicate clearly and effectively with strong organizational skills and the ability to interface effectively with all levels of the organization.
- Confidentiality: Ability to maintain a strict standard of confidentiality in handling sensitive information, reports, and correspondence.
- Conflict resolution: Ability to solve problems and resolve issues with sound judgment under stressful situations.
- Interpersonal skills: Interacts with employees' contractors, customers, community, family members, referral sources, governmental sponsoring and funding entities, program auditors, advocacy groups, and the media. - Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
- Task management: Ability to perform tasks effectively and efficiently and to handle multiple priorities in a confidential environment. Must have strong attention to detail. OTHER REQUIREMENTS OR CONDITIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
- Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
- Must have computer experience to include Microsoft Word, Microsoft Edge, Adobe Acrobat, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax).
- Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
- Must maintain regular attendance and reliability 100% of the time to ensure consistency and completeness of program's processes, as this is critical to business operations. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm, or agreed-upon hours. Evening and weekend work may be required as job duties demand.
