JOB TITLE: TCOOMMI ICM Case Manager

LOCATION:  Victoria, Texas

JOB PURPOSE: Provides intensive case management services to offenders with severe and persistent mental illness who are on adult criminal justice supervision (probation or parole) within our service area with the goal of reducing recidivism and successful completion of probation or parole through management of mental health related problems. Services will be provided in the community, clinic, and will include at least one monthly contact with the assigned supervising officer.  Is responsible for completing assessments, recovery planning, and effective coordination of appropriate community services.  Works closely with the criminal justice system and acts as liaison between offenders and supervising officer. Requires travel within the service counties.  Meets multiple deadlines and reporting requirements.

ESSENTIAL FUNCTIONS and DUTIES: (1) Completes TRR assessment and Recovery Plan in accordance with Center policy, TCOOMMI Contract, and DSHS Contract. (2) Provides Intensive Case Management services to offenders according to the recovery plan, TCOOMMI ICM PGP's, and Medicaid Rule.  (3) Ensures each offender receives a minimum of 3.5 face-to-face hours of service per month to include services provided by case manager, nursing, psychiatrist, and benefits specialists.  (4) Provides ninety five percent (95%) of intensive case management services in the consumers' natural environment.  (5) Submits treatment recommendations through Utilization Review based on medical necessity.  (6) Documents services collaboratively according to Medicaid Rule and TCOOMMI PGP's.  (7)  Completes all paperwork according to Medicaid Rule and Center policy.  (8) Submits documentation of services within 2 business days of service delivery.  (9) Attends all required team meetings.

LEVEL OF SUPERVISION REQUIRED:  General

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY: Must have computer experience to include, Microsoft Word, Excel, and Outlook.  Must have good organizational skills to achieve multiple deadlines. Exhibits cooperative teamwork with other staff to accomplish the efficient operation of the program.  Models professionalism by appropriate dress, language, ethics and work habits.  

EDUCATION AND EXPERIENCE REQUIRED:

Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse.  (Social, behavioral, and human services are:  psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred: At least six months experience in providing mental health services to adults, children and adolescents.

DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE): BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
