POSITION TITLE: Mobile Crisis Outreach Team Case Manager

WORKSITE LOCATION: GBC Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

STARTING PAY: $18.47/Hourly  EEOC CODE: Professional FLSA STATUS: Non-Exempt

SHIFT: Fulltime; Monday through Friday 8am - 5pm, with rotating on-call shifts that cover evenings, nights, and weekends

LEVEL OF SUPERVISION REQUIRED: General

The mission of Gulf Bend Center (GBC) is to improve the quality of life in our community for individuals and their families by providing excellent and trusted care for wellness. GBC is committed to providing quality person-centered, trauma-informed care to individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.

JOB PURPOSE:

This position will provide crisis intervention services to reduce symptoms of severe and persistent mental illness or serious emotional disturbance and to prevent admission of an individual to a more restrictive environment. Will work in cooperation with family members and community organizations including, but not limited to law enforcement, hospital staff, and other community providers. On-call crisis response duties are included in this role as part of a scheduled rotation. Requires travel within in Gulf Bend Center's (7) county catchment area.

ESSENTIAL FUNCTIONS and DUTIES:

  1. Responds or makes contact with crisis event within contract timeframe of 1 hour.
  2. Thoroughly completes a crisis log with supporting documentation.
  3. Submits all documentation within center policy.
  4. Provides crisis services per HHSC contract, Medicaid Rule, and Center policy.
  5. Will complete 7 day follow up on hospital discharges per HHSC contract.
  6. Will participate in an after-hour call rotation to include holidays and weekends.
  7. Will provide LOC5 services as assigned.
  8. Will assist with intake screenings as needed.
  9. Provides crisis services in the community, clinic, or local emergency room departments.
  10. Must maintain regular and reliable attendance.
  11. Complies with telework/remote agreement.
  12. Uses the trauma informed care approach when providing services and working with others.
  13. Understands the Three E's of trauma-informed care.
  14. Understands the Four R's of trauma-informed care.
  15. Understands the six key principles of trauma-informed care.

Trauma-informed:

A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

EQUIPMENT, TOOLS AND MATERIALS

  1. Computer, telephone, projectors, calculator, printer, fax machine, and copy machine are used in performance of essential job duties.
  2. Various forms of paperwork are handled in the performance of essential job duties.

EDUCATION AND EXPERIENCE REQUIRED:

Required: A bachelor's degree from an accredited college or university and minimum number of hours equivalent to a major in social, behavioral or human services or is a registered nurse or is an LPHA. (Social, behavioral, and human services: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human growth and development; physician assistant; gerontology; special education; educational psychology; early childhood intervention; or early childhood education.) Knowledge of DSM V mental disorders.

Preferred: At least six months' experience in providing mental health services to adults, children, and adolescents.

DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE):

  1. BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas or LPHA.

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  3. Good driving record as defined by Gulf Bend Center.
  4. Must complete all CORE training requirements prior to the due date.
  5. Must have computer experience to include Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  6. Must maintain regular attendance and reliability which is critical to business operations. This is a full-time position. Day and night shifts with rotating on-call evenings and weekends or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
