SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm, with some variation to meet job requirements and customer needs

JOB PURPOSE:

Preadmission Screening and Review (PASRR) is a federally mandated program. PASRR QMHP-CS position will ensure that any individual seeking admission into a Medicaid-certified nursing facility or currently residing in such a facility is screened and evaluated for mental illness, (or where applicable intellectual disability, and developmental disabilities). Purpose of the evaluation is to determine whether an individual identified as having mental illness needs specialized services, to identify alternate placement options when necessary or requested, and to identify specialized services that may benefit the individual. Must meet multiple deadlines and reporting requirements. This position will also provide routine case management, skills training, and/or psychosocial rehab services to adults with severe and persistent mental illness. Services will be provided in the nursing facility setting to individuals shown as positive for mental health services after an evaluation is conducted. Is responsible for completing assessments, treatment planning, effective coordination of appropriate community services and crisis intervention. Acts as liaison between consumers/families and service providers. Requires travel within the 7 counties. May perform other work duties as assigned by Manager of IDD Services. Work is performed under the general supervision of the PASRR Team Lead with considerable latitude for the exercise of discretion and independent judgment.

ESSENTIAL FUNCTIONS and DUTIES:

QMHP-CS must have working knowledge of the nature of serious mental illnesses and related treatment modalities, interventions, and techniques. Need to be familiar with the different types of assessments and their uses in the recovery planning process. Knowledge of consumer rights, local community resources and service delivery systems. Duties include but are not limited to completing case management assessments, developing person-centered recovery plans, and providing skills-training and routine case management to help consumers achieve their goals and objectives. QMHP-CS is responsible for meeting with individual to build rapport and participate in integrated care planning activities to improve and enhance service continuity and effectiveness. Utilize effective listening skills to establish connection with client, family members and other individuals close to the client; provide emotional support and encouragement and engage family members in the service process.

  1. Complete a PASRR Evaluation (PE) within required timeframes to determine if the person has an MI, ID or DD diagnosis and, if so, to:
  2. assess the person's need for care in an NF;
  3. assess the person's need for specialized services; and
  4. identify alternate placement options.
  5. Administer the Texas uniform assessment tool (Adult Needs and Strengths Assessment-ANSA) to determine level of care and service planning to facilitate quality improvement initiatives and to allow for outcomes monitoring.
  6. Accepts newly assigned cases in which the diagnostic intake has been endorsed by a licensed provider.
  7. Develops Person-Centered Recovery Plans with consumers as the means for implementing appropriate services and developing effective rapport and outcomes with consumers.
  8. Provides on-going case management functions in accordance with the consumer's needs and the strategies identified within the service plan to help the consumers achieve goals and objectives.
  9. Participates in integrated care planning and care coordination activities as directed to improve and enhance service continuity and effectiveness for consumers.
  10. Maintains close communication with the nursing facility psychiatric services (where applicable), social services, and nursing, for input regarding medication compliance, side effects of medication, and medication changes. Alerts these members of the staff of any changes in customer adjustment which might suggest decompensation and a need for more aggressive intervention.
  11. Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
  12. Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.

  1. Responsible for providing skill building training relating to goals and objectives using evidence-based practices and curriculum.
  2. Maintains all client documentation in the electronic record and write progress notes that are adhering to requirement set by the Texas Administrative Code. All service delivery notes entered within 2 business days.
  3. Maintains records in accordance with regulatory/agency standards. Enters data in a clear, accurate, and timely manner. Also, promptly submits documentation needed for the tracking of output/outcome measures. May be asked to assist in record review or qualitative review processes.
  1. Participate in staff meetings, monthly training, company programs, professional development training as required.

Trauma-informed:

A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

EDUCATION AND EXPERIENCE REQUIRED:

Required: Qualified Mental Health Professional-Community Services (QMHP-CS) - A staff member who is credentialed as a QMHP-CS who has demonstrated and documented competency in the work to be performed and:

  • has a bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major (as determined by the LMHA or MCO in accordance with TAC §412.316(d) of this title, relating to Competency and Credentialing) in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention.
  • is a registered nurse.
  • completes an alternative credentialing process as determined by the LMHA or MCO in accordance with TAC §412.316(c) and (d) of this title relating to (Competency and Credentialing).

Preferred: Experience working directly with individuals with mental illness.

DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE):

Bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major (as determined by the LMHA or MCO in accordance with TAC §412.316(d) of this title, relating to Competency and Credentialing) in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education or early childhood intervention; or, is a registered nurse.

Ability to obtain Adult Needs and Strengths Assessment-ANSA certification.

  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  3. Good driving record as defined by Gulf Bend Center.
  4. Must complete PASRR required trainings through Texas Health and Human Services Learning Portal
  5. Must complete all CORE training requirements prior to the due date.
  6. Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  7. Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
