POSITION TITLE: Case Manager, Pathway

DEPARTMENT: Crisis & Diversion Services     REPORTING UNIT: MHM 27

WORKSITE LOCATION: Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

STARTING PAY: $18.47/hr     EEOC CODE: Professional     FLSA STATUS: Non-Exempt

SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)


ESSENTIAL FUNCTIONS AND DUTIES:
1. Works as a team member with Crisis Services and TRR Case Management Services.
2. Will manage the Pathway program to include assessment for suicide risk, safety planning, and monitoring the individuals in the program.
3. Will provide crisis intervention services if needed.
4. Will provide crisis services per HHSC contract, Medicaid Rule, and Center policy.
5. May assist in providing continuity of care services complete hospital followups in accordance with our State contract as needed.
6. May assist with intake screenings as needed.
7. Will work in cooperation with family members and community organizations including, but not limited to law enforcement, hospital staff, and other community providers.
8. Provides services in the community, clinic, local emergency room departments, or the customer's natural environment.
9. Will require travel within Gulf Bend Center's 7 county catchment area.
10. Attends all required team meetings as scheduled by supervisor unless excused by supervisor.
11. Demonstrates efficient time management as evidenced by meeting all job expectations.
12. Documents services collaboratively according to Medicaid Rule and HHSC Contract per supervisory review.
13. Completes all paperwork according to Medicaid Rule, HHSC Contract and Center Policy.
14. Submit documentation of services within two business days.
15. Assists in managing Pathway caseload and program spreadsheet.
16. May assist in policy and procedure development of program as needed.
17. Must maintain regular and reliable attendance.
18. Complies with telework/remote agreement.
19. Uses the trauma informed care approach when providing services and working with others.
- Understands the Three E's of trauma-informed care.
- Understands the Four R's of trauma-informed care.
- Understands the six key principles of trauma-informed care.


EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred: At least six months' experience in providing mental health services to adults, children and adolescents preferred.

DEGREE AND/OR CERTIFICATION REQUIRED (IF APPLICABLE):

  1. BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

OTHER REQUIREMENTS OR CONDITIONS (specify):

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  3. Good driving record as defined by Gulf Bend Center.
  4. Must complete all CORE training requirements prior to the due date.
  5. Must have computer experience to include Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  6. Must maintain regular attendance and reliability, which is critical to business operations. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of the program's processes. Evening and weekend work may be required as job duties demand.
