JOB TITLE: Pathway Case Manager
WORKSITE LOCATION: Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
STARTING PAY: $18.47/HR EEOC CODE: Professional FLSA STATUS: Non-Exempt
SHIFT: Fulltime; Monday through Friday, 8:00am to 5:00pm
JOB PURPOSE:
Provides services to individuals identified at risk for suicide and assigned to the Suicide Safe Care Pathway program. Completes Columbia Suicide Severity Rating Scale (C-SSRS) safety planning, and effective coordination of appropriate community services to reduce symptoms of severe and persistent mental illness or serious emotional disturbance and to prevent admission of an individual to a more restrictive environment as needed.
EDUCATION AND EXPERIENCE:
Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: At least six months' experience in providing mental health services to adults, children and adolescents preferred.
DEGREE AND/OR CERTIFICATION REQUIRED:
BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
ESSENTIAL FUNCTIONS AND DUTIES:
2. Collaborates and cooperates with other GBC staff, community organizations, and the individual's family/collateral as needed.
3. Completes C-SSRS, CALM check list, and safety plan at every visit.
4. Refers and links customers to other resources and services in the community as needed.
5. Completes Hospital follow-up services as needed.
6. Completes Intake Screenings as needed
7. Participates in team meetings as scheduled by supervisor
8. Completes all paperwork according to Center Policy and HHSC ZEST.
9. Assists in managing the Pathway caseload and program tracking.
10. Completes other work duties as assigned by Supervisor
11. Provides services in the community, clinic, and/or the customer's residence.
12. Provides information and feedback to assist in meeting contract measures as requested.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY:
1. Program specific: Assists in tracking and monitoring caseload of individuals assigned to the Pathway. Requires travel within the 7 counties. Work is performed under the general supervision of the Program Manager with considerable latitude for the exercise of discretion and independent judgment.Exhibits cooperative teamwork with other staff to accomplish the efficient operation of the program.

Ability to work cooperatively and productively with supervisors, individuals, co-workers, and groups of people at all levels of activity, contributing to a strong spirit of teamwork. 10. Fleet: Completes fleet checklist as required. Report all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
