POSITION TITLE: Substance Use Intensive Case Manager
WORKSITE LOCATION: Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
STARTING PAY: $19.69/hr EEOC CODE: Professional FLSA STATUS: Non-exempt
SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm, some extended shifts evenings possible
EDUCATION AND EXPERIENCE REQUIRED: Bachelor's degree from an accredited college or university with a major in social, behavioral or human services or a related field of study and a minimum of two (2) years' equivalent experience providing case management services.
ESSENTIAL FUNCTIONS and DUTIES:
1. Knowledge of chemical dependency and alcoholism, addiction, co-dependency, and related disorders; of treatment techniques; and of recovery processes.
2. Performs case management and connect customers with needed referrals such as medical, dental, mental health, education, employment, social services, and other community services. Maintains rapport with and knowledge of local community resources to ensure effective referral options and contacts.
3. Performs individual sessions with customers in accordance with treatment plans, or as necessary for crisis intervention. Provides customers with feedback and support and assists customers in addressing behaviors with family, social, or personal problems. Maintains communication with other departments of Gulf Bend to ensure continuity of care and integrative services.
4. Performs Substance Use skills training group sessions as scheduled and follows curriculum to meet fidelity guidelines.
5. Ability to write progress notes according to standard, assessments, memos, letters, various complex reports and miscellaneous other documents in accordance with guidelines and with acceptable spelling and grammar.
6. Develops customer discharge plans that integrate aftercare treatment and utilization of appropriate referral sources. Coordinates discharges with social service agencies and community organizations as appropriate.
7. Ability to complete instructions, communicate in a courteous and effective fashion with co-workers, customers and families, agencies and the public. Displays a customer service orientation at all times treating customers as equals and partners.
8. Ability to work within a team to develop service plans, coordinate service delivery and monitor customer progress.
9. Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office).
10. May perform other work duties as assigned by Specialty Clinical Services Manager.
11. Must maintain regular and reliable attendance.
12. Complies with telework/remote agreement.
13. Uses the trauma informed care approach when providing services and working with others.
- Understands the 3 E's of trauma-informed care: Events, Experiences, and Effects.
- Understands the 4 R's of trauma-informed care: Realization, Recognize, Respond, Resist Re-traumatization..
- Understands the 6 key principles of trauma-informed care.
EDUCATION AND EXPERIENCE REQUIRED: Bachelor's degree from an accredited college or university with a major in social, behavioral or human services or a related field of study and a minimum of two (2) years' equivalent experience providing case management services.
DEGREE AND/OR CERTIFICATION REQUIRED: Bachelor's Degree in social, behavioral or human services or a related field of study.
