JOB TITLE: YES Waiver, Child and Adolescent Services Case Manager

SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm
EDUCATION AND EXPERIENCE REQUIRED:
Required: A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human growth and development; physician assistant, gerontology; special education; educational psychology; early childhood education; or early childhood intervention or is a registered nurse.
Preferred: At least six months' experience in providing mental health services to adults, children and adolescents.
DEGREE AND/OR CERTIFICATION REQUIRED:
BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
ESSENTIAL FUNCTIONS and DUTIES:
- Completes TRR assessment in accordance with Center policy and HHSC Contract as verified by supervisory review. Initial TRR assessments are completed within 7 days of assignment and must be updated at least two weeks prior to expiration for continuity of care.
- Formulates a plan of care to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the TRR assessment according to TAC code, HHSC contract, and National Wraparound Implementation Center (NWIC) model.
- Provides authorized services to consumers according to the Recovery plan, Wraparound plan, and/or Individual Plan of Care (IPC) developed using independent clinical judgment in accordance with Medicaid Rule, HHSC Contract, National Wraparound Implementation Center (NWIC) model and UM Guidelines..
- Provides ninety-five percent (95%) of services in the customers' natural environment (including at home, school, and community).
- Meets productivity standard.
- Submits treatment recommendations through Utilization Review based on medical necessity per supervisor review.
- Actively participates in all team meetings, Wraparound coaching, and supervisions as a means of enhancing professional growth, reviewing and processing the provision of services, and appropriately addressing concerns and other requests as needed.
- Submits all documentation in the electronic healthcare record (EHR) within two business days including forms, services, disclosures, telephone contacts etc. within 2 business days.
- Monitors all services for YES Waiver in accordance with Individual Plan of Care (IPC) and YES Waiver Policy Manual.
- Plans, coordinates, and facilitates at least monthly Child and Family Team meetings for customers in a Wraparound level of care.
- Demonstrates appropriate responses to Critical Incidents: submits Critical Incident Reports to HHSC, facilitates Child and Family Team meetings, and adjusts Crisis and Safety plan as needed.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY:
- Program specific: Completes NWIC trainings for Wraparound Facilitators and maintains knowledge and skills in Wraparound Planning process. Is responsible for completing assessments, Wraparound planning, treatment planning, effective coordination of appropriate community services, and crisis intervention. Acts as liaison between customers/families and service providers. Requires travel within the 7 counties. Meets multiple deadlines and reporting requirements. Work is performed under the supervision of the Program Manager, Child and Adolescent Services with considerable latitude for the exercise of discretion and independent judgment. Performs other duties as assigned by supervisor which are consistent with the position and in compliance with agency policies and procedures. Exhibits cooperative teamwork with other staff to accomplish the efficient operation of the program by assisting coworkers, as necessary, to meet client needs and contract requirements with no verified complaints. Demonstrates ability to engage customers and families in treatment and lead Child and Family Team meetings.

- Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
- Task management: Ability to perform tasks effectively and efficiently and to handle multiple priorities in a confidential environment. Must have strong attention to detail. OTHER REQUIREMENTS OR CONDITIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
- Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
- Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax).
- Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
- Must maintain regular and reliable attendance, including compliance with the telework agreement. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of the program's processes. Evening and weekend work may be required as job duties demand.
