POSITION TITLE: Continuity of Care Case Manager, Community Response Team

WORKSITE LOCATION: Victoria County Jail (primary)

STARTING PAY: $18.47 EEOC CODE: Professional FLSA STATUS: Non-Exempt

SHIFT: Fulltime; Monday thru Friday, 9:00am-6:00pm

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)


ESSENTIAL FUNCTIONS and DUTIES:
1. Responds to referrals and/or notifications from the jail(s) within 2 business days.
2. Identifies if inmate is admitted to GBC per clinical system.
3. Assesses inmate to identify current needs.
4. Provides follow up as needed while detained in jail.
5. Documents daily in spreadsheet to track the number of inmates and outcome of visit.
6. Effectively communicates with case managers on all current GBC clients that are receiving COC services in jail.
7. Provides follow up in the community as needed when inmates are released.
8. Coordinates with jail staff for visits with inmates and release date.
9. Responds and assesses an individual screening “high risk” for mental health needs prior to entering jail within timeframe of 1 hour.
10. Responds and/or makes contact with a crisis event within timeframe of 1 hour.
11. Thoroughly completes a crisis log with supporting documentation.
12. Submits all documentation of services within two business days.
13. Participates in Collaborative Meetings and has data prepared as needed.
14. Meets with community outreach providers to build relationships and educate on COC services.
15. Coordinates and links individual to community supports and services to meet their needs.
16. Answers inquiries by providing accurate and appropriate responses in writing, via e-mail or telephonic.
17. Responds to court ordered assessments and thoroughly completes crisis assessment with supporting documentation and submits back to magistrate within timeframe.
18. Must maintain regular and reliable attendance.
19. Demonstrates efficient time management as evidenced by meeting all job expectations.
20. Complies with telework/remote agreement.
21. Uses the trauma informed care approach when providing services and working with others.
• Understands the Three E’s of trauma-informed care.
• Understands the Four R’s of trauma-informed care.
• Understands the six key principles of trauma-informed care.

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.) Knowledge of DSM-IV/V disorders useful.

Preferred: At least six months experience in providing mental health services to adults, children and/or adolescents. Work experience working within jail setting.

REQUIREMENTS OF TEXAS CJIS SYSTEM ACCESS:

Required: Applicant must meet the below criteria to be able to access the CJIS system within the justice department:

Permanent Disqualification includes:

  • FELONY CONVICTION
  • FELONY DEFERRED ADJUDICATION
  • CLASS A MISDEMEANOR CONVICTION
  • CLASS A MISDEMEANOR DEFERRED ADJUDICATION
  • FAMILY VIOLENCE CONVICTION OR DEFERRED ADJUDICATION

Cannot have within the last 10 years:

  • CLASS B MISDEMEANOR CONVICTION
  • CLASS B MISDEMEANOR DEFERRED ADJUDICATION

Disqualified until disposition given:

  • OPEN ARREST FOR ANY CRIMINAL OFFENSE (FELONY OR MISDEMEANOR)

* Offenses that were committed as a juvenile will receive the same consideration and will be held to the same standard as adult offenses.

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  3. Good driving record as defined by Gulf Bend Center.
  4. Must complete all CORE training requirements prior to the due date.
  5. Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
  6. Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 2:00pm to 11:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes. Evening and weekend work may be required as job duties demand.
