POSITION TITLE: Licensed Vocational Nurse
WORKSITE LOCATION: Gulf Bend Center Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
PAY GRADE: 15 EEOC CODE: Professional FLSA STATUS: Non-Exempt
SHIFT: Fulltime during Clinic hours, Monday thru Friday, 8am to 5pm, some evenings
EDUCATION AND EXPERIENCE REQUIRED:
Required: A vocational degree from an accredited vocational college with a major in Vocational Nursing.
Preferred: At least one year experience in providing mental health services to adults, children and adolescents.
JOB PURPOSE:
LVN-Mental Health Clinic works with the Mental Health Practitioner to deliver services to children, adolescents, and adults in a clinic setting. The LVN will function within the scope of practice to ensure the client's needs are met in the clinical setting.
ESSENTIAL FUNCTIONS and DUTIES:
- Collect vital signs, collect medical and medication history, and collect any other needed medical information from the individuals seen in the clinic.
- Perform any other relevant duties associated with an individual's clinic visit, such as communicating with the individual's pharmacy of choice, completing prior authorizations, completing referrals to higher level of care, CLIA waived testing, performing lab draws, obtaining lab and radiology results, and assisting the Mental Health Practitioner with educating customers on their medication and symptom management.
- Perform pill packing education and supervision for customers with an identified need.
- Administer intramuscular medications as needed.
- Provide outreach nursing services (if needed) to individuals in their home including pill packs, injections, lab draws, education on medication management, symptom management and psychosocial rehabilitation.
- Address all concerns about the individual's illness with the treatment team. The LVN may be asked to perform chart audits.

OTHER REQUIREMENTS OR CONDITIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
- Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
- Good driving record as defined by Gulf Bend Center.
- Must complete all CORE training requirements prior to the due date.
- Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.
- Must maintain regular attendance and reliability which is critical to business operations to ensure consistency and completeness of program's processes. The Clinic is open Monday through Friday, 8:00am to 5:00pm; evening and weekend work may be required as job duties demand.
