WORKSITE LOCATION: Gulf Bend Center Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

STARTING PAY: $18.69/HR EEOC CODE: Technician FLSA STATUS: Non-Exempt

SHIFT: Fulltime during Clinic hours, Monday through Friday, 8am to 5pm, some rotating evenings for late Clinic.

JOB PURPOSE:

Licensed Vocational Nurse works primarily in the Mental Health Clinic with the Mental Health Practitioner to deliver services to children, adolescents, and adults in a clinic setting. The LVN will function within the scope of practice to ensure the customers' needs are met in the clinical setting.

EDUCATION AND EXPERIENCE:

Required: A vocational degree from an accredited vocational college with a major in Vocational Nursing.

Preferred: At least one year's experience in providing mental health services to adults, children and adolescents.

DEGREE AND/OR CERTIFICATION REQUIRED:

Licensed Vocational Nurse in the state of Texas.

ESSENTIAL FUNCTIONS and DUTIES:

  1. Collects vital signs, physical and mental health history, medication history, and any other needed medical information from the individuals seen in the clinic.
  2. Practices Nursing with integrity.
  3. Performs any other relevant duties associated with an individual's clinic visit, such as communicating with the individual's pharmacy of choice, completing prior authorizations, completing referrals to higher level of care, Perform CLIA waived testing, lab draws and obtaining lab and radiology results. Assist the Mental Health Practitioner with educating customers on their medication and symptom management.
  4. Performs pill packing education and supervision for customers with an identified need.
  5. Administers intramuscular medications as needed and monitors for any adverse reactions.
  6. Provides outreach nursing services, if needed, to individuals in their home including pill packs, injections, lab draws, education on medication management, symptom management and psychosocial rehabilitation.
  7. Addresses concern about the individual's illness with the treatment team. May be asked to perform chart audits.
  8. Travels to Satellite Clinics and work late Clinic following a rotating schedule.
  9. Assists with Patient Assistance Program when needed.
  10. Maintains accurate and timely documentation in compliance with clinic policy, HIPPA, and legal requirements.
  11. Support clients during a crisis situation and implement de-escalation techniques when necessary.

  9. Fleet: Completes fleet checklist as required. Report all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.

OTHER REQUIREMENTS OR CONDITIONS:

  1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
  3. Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax).
  4. Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
  1. Must maintain regular and reliable attendance, including compliance with the telework agreement. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of the program's processes. Rotating evenings for late clinic is required as job duties demand.
