POSITION TITLE: Case Manager, School-Based Services
WORKSITE LOCATION: VISD school campus, Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
PAY RANGE: $18.47 - $20.32 EEOC CODE: Professional FLSA STATUS: Non-Exempt
SHIFT: Fulltime; Monday thru Friday, 7:30am - 4:30pm
The mission of Gulf Bend Center (GBC) is to improve the quality of life in our community for individuals and their families by providing excellent and trusted care for wellness. GBC is committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.
JOB PURPOSE:
Provides services to children/adolescents with identified needs as part of early intervention support toward developing the whole child. Also provides case management, skills training, and medication training to children/adolescents with a serious emotional disturbance. Provides services to the primary caregiver or LAR. Services will be provided in the school setting and/or community to increase accessibility and engagement to necessary services in rural communities. Is based on a school campus and integrated with school personnel. Collaborates and partners with School Counselor. Social Emotional Behavior Support Specialist (SEBS), and Administrators at the school. Is responsible for completing assessments, recovery planning, effective coordination of appropriate community services, and crisis intervention. Meets multiple deadlines and reporting requirements.
ESSENTIAL FUNCTIONS and DUTIES:
- Collaborates with school personnel to meet the needs of the child and family, the school, and community.
- Attends school meetings and assists in educating school personnel in mental health topics.
- Exhibits cooperative teamwork with other Center staff, school campus personnel, and school district personnel to accomplish the efficient operation of the program by assisting team members, as necessary, to meet client needs, MOU, and contract requirements with no verified complaints.
- Completes Crisis Intervention when needed.
- Completes TRR
assessment and Recovery Plan in accordance with Center policy and HHSC Contract as verified by supervisory review.
- Formulates a Recovery plan to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the TRR
assessment as verified by supervisory review.
- Provides authorized services to consumers according to the recovery plan developed using independent clinical judgment to assess client need per supervisor review.
- Provides majority of services in the school setting and/or customer's natural environment.
- Meets Center's productivity standards monthly per Program Director's review.
- Links customers and families to appropriate community resources as needed to meet their needs
- Provides customers Patient and Family Education in accordance with TRR
Fidelity per supervisory review.
- Provides case management, skills training, and medication training in accordance with Medicaid Rule, HHSC Contract, and TRR Fidelity.
- Demonstrates appropriate responsiveness to consumer needs as demonstrated by no legitimate consumer, collateral or co-worker complaints.
- Submits treatment recommendations through Utilization Review based on medical necessity per supervisor review.
- Attends and participates in all required team meetings as scheduled by supervisor unless excused by supervisor.
- Demonstrates efficient time management as evidenced by meeting all job expectations.
- Documents services collaboratively according to Medicaid Rule and HHSC Contract per supervisory review.
- Completes all paperwork according to Medicaid Rule, HHSC Contract and Center Policy.
- Submits documentation of services within two business days.
- Models professionalism by appropriate dress, language, ethics and work habits 100% of the time.
- Communicate effectively both orally and in writing 100% of the time.
- Uses a computer and other technical devices to complete all work assignments.
- Accomplishes 95% of assigned tasks with minimal supervision.
- Uses sound judgment in the release of information as evidenced by lack of complaints or consumer rights findings.
- Follows fleet policy including pre- and post- inspections. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem.
- Maintains regular and reliable attendance.
- Complies with telework/remote agreement.
- Uses a Trauma Informed Care approach when providing services and working with others.

Trauma-informed:
A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

EQUIPMENT, TOOLS AND MATERIALS
- Computer, telephone, cell phone, projectors, calculator, fax machine, printer, copy machine are used in performance of essential job duties.
- Various forms of paperwork and computer programs are handled in performance of essential job duties.
EDUCATION AND EXPERIENCE REQUIRED: BA/BS degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.) Preferred: At least six months experience in providing mental health services to adults, children and adolescents.

OTHER REQUIREMENTS OR CONDITIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Must maintain personal liability insurance.
- Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
- Good driving record as defined by Gulf Bend Center.
- Must complete all CORE training requirements prior to the due date.
- Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines.
- Ability to type 40 wpm.
- Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 7:30am-4:30pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
