JOB TITLE: Case Manager, School-Based Services
WORKSITE LOCATION: VISD school campus and Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
STARTING PAY: $18.47/HR EEOC CODE: Professional FLSA STATUS: Non-Exempt
SHIFT: Fulltime; Monday through Friday, 7:30am to 4:30pm on location during school session; Monday through Friday 8:00am to 5:00pm in office summer and holidays
JOB PURPOSE:
Provides services to children/adolescents with identified needs as part of early intervention support toward developing the whole child; case management, skills training, and medication training to children/adolescents with a serious emotional disturbance; and provides services to the primary caregiver or LAR. Based on a school campus and integrated with school personnel and provides services in the school setting and/or community to increase accessibility and engagement to necessary services in rural communities. Collaborates and partners with School Counselor. Social Emotional Behavior Support Specialist (SEBS), and Administrators at the school.
EDUCATION AND EXPERIENCE:
Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: At least six months experience in providing mental health services to adults, children and adolescents.
DEGREE AND/OR CERTIFICATION REQUIRED:
BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
ESSENTIAL FUNCTIONS and DUTIES:
2. Provides School Based Case Management services according to Center Policy and Contract requirements
3. Provides Crisis Intervention services according to HHSC Contract and Center Policy.
4. Provides case management, skills training, and medication training in accordance with Medicaid Rule, HHSC Contract, and UM Guidelines
5. Completes CANS assessment in accordance with Center policy and HHSC Contract.
6. Formulates a plan of care to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the CANS.
7. Provides ninety-five percent (95%) of services in the consumers' natural environment (including at home, school, and community).
8. Meets productivity standard.
10. Submits all documentation including forms, services, disclosures, and telephone contacts etc within two (2) business days.
11. Tracks and manages caseload to avoid expiration of Consumer's required documents and forms to avoid lapse in services.

7. 8. Customer service: Demonstrates appropriate responsiveness to internal/external customer needs.
9. Dress code: Adherence to Center Dress Code to present a professional appearance and conduct when representing the Center.
10. Fleet: Completes fleet checklist as required. Report all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
OTHER REQUIREMENTS OR CONDITIONS:
1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
4. Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
5. Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 7:30am-4:30pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes.
