SHIFT: Fulltime; Monday through Friday, 8:00am to 5:00pm
JOB PURPOSE:
Provides intensive case management services to offenders with severe and persistent mental illness who are on adult criminal justice supervision (probation or parole) within our service area with the goal of reducing recidivism and successful completion of probation or parole through management of mental health related problems.
EDUCATION AND EXPERIENCE:
Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)
Preferred: At least six months' experience in providing mental health services to adults, children and adolescents; work experience within jail setting.
DEGREE AND/OR CERTIFICATION REQUIRED:
BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.
ESSENTIAL FUNCTIONS and DUTIES:
2. Provides Intensive Case Management services to offenders according to the recovery plan, TCOOMMI ICM PGPs, and Medicaid Rule.
3. Ensure each offender receives a minimum of 3.5 face-to-face hours of service per month to include services provided by case manager, nursing, psychiatrist, and benefits specialists.
4. Provides ninety-five percent (95%) of intensive case management services in the consumers' natural environment.
5. Submits treatment recommendations through Utilization Review based on medical necessity.
6. Documents services collaboratively according to Medicaid Rule and TCOOMMIPGP's.
7. Complete all paperwork according to Medicaid Rule and Center policy.
8. Submits documentation of services within 2 business days of service delivery.
9. Works in collaboration with the supervising officer to help stabilize mental health symptoms and reduce the risk of recidivism.
10. Facilitates quarterly Interdisciplinary Team meetings with offender, Case Manager, supervising officer, and program director/designee.
11. Submits incident reports to TCOOMMI within contract required timeframes.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY:
- Program specific: Responsible for completing assessments, recovery planning, and effective coordination of appropriate community services. Works closely with the criminal justice system and acts as liaison between offenders and supervising officers. Requires travel within the service counties. Meets multiple deadlines and reporting requirements. Services will be provided in the community or clinic and will include at least one monthly contact with the assigned supervising officer.

- Task management: Ability to perform tasks effectively and efficiently and to handle multiple priorities in a confidential environment. Must have strong attention to detail. - Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
OTHER REQUIREMENTS OR CONDITIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
- Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
- Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax).
- Must complete all training requirements prior to the due date. Attend required training and team meetings. Relias training and any other training required for the role is 100% compliant.
- Must maintain regular and reliable attendance, including compliance with the telework agreement. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of the program's processes. Evening and weekend work may be required as job duties demand
