JOB TITLE: TXHML Service Coordinator

JOB LOCATION:  Gulf Bend Plaza, 6502 Nursery Drive, Victoria TX  77904

COMPETITIVE SALARY BASED ON EXPERIENCE:  Pay Grade:  13, minimum:  $ 16.33/hour; Fulltime, M-F, 8am-5pm

JOB PURPOSE: Provides services to adults, adolescents and children with Intellectual and Developmental Disabilities, primarily to individuals who are eligible/receiving Texas Home Living Services (TXHML) and General Revenue (GR). Provides services in the office, in the home or at other locations as needed. Provides the day to day service coordination for individual and their families in the TXHML and General Review Program. Completes all documentation related to services provided.  Work is performed under the general supervision of the Service Coordinator Manager with considerable latitude for the exercise of discretion and independent judgment.

ESSENTIAL FUNCTIONS and DUTIES: Duties include meeting with the Service Planning Team (SPT) to develop the person directed plan, linking, assessing and coordinating to available community resources.  Other duties include monitoring all services that are provided by the TXHML Waiver program and other non-waiver services that individual is receiving. Service Coordinators will collaborate with existing behavioral health providers, and other community and local service resources, traditional and non- traditional, to assist the individual in achieving positive outcomes. Services will complete Annual Reviews, monthly contact visits with individuals and contacts with providers, medical or other individuals who are involved in their treatment plan. Understand trauma informed services.

LEVEL OF SUPERVISION REQUIRED:  General

KNOWLEDGE, SKILLS ABILITIES and COMPETENCY:  

Trauma Informed Care:  Gulf Bend Center incorporates principles of trauma-informed care, which includes cultural sensitivity, in all areas of service delivery.  All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position.

Must have computer experience to include, Microsoft Word, Excel, and Outlook.  Must have good organizational skills to achieve multiple deadlines.

EDUCATION AND EXPERIENCE REQUIRED:

Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse.  (Social, behavioral, and human services are:  psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred: At least six months experience in providing mental health or intellectual disability services to adults, children or adolescents.

DEGREE AND/OR CERTIFICATION REQUIRED:  BS/BA or RN

PHYSICAL AND ENVIRONMENTAL REQUIREMENTS: Light lifting, under 15 lbs, light carrying, under 15 lbs, straight pulling, simple grasping, walking, standing, sitting, operating office equipment, operating motor vehicle, ability to see, identify colors, hearing (with aid), ability to write, ability to count, ability to read, ability to tell time, travel by car 35% of time, outside, inside, working closely with others, working alone.

OTHER REQUIREMENTS OR CONDITIONS: Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.  Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must complete all CORE training requirements in month which they are due.
