Team Lead, Service Coordinator

WORKSITE LOCATION:  Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

PAY RANGE:  $20.00 - $22.00 minimum  EEOC CODE:  Professional  FLSA STATUS:  Non-exempt 

SHIFTFulltime; Monday thru Friday, 8:00am - 5:00pm, with some variation to meet job requirements and customer needs

EDUCATION AND EXPERIENCE REQUIRED:  A bachelor's degree from an accredited college or university with a major in social, behavioral, or human services or is a registered nurse.  (Social, behavioral, and human services are:  psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education).  At least 1-year experience providing service coordination to individuals with Intellectual Developmental Disabilities or equivalent experience with LIDDA services and supports.

DEGREE AND/OR CERTIFICATION REQUIRED:  BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

JOB PURPOSE:  This position will act as a Team Leader to the IDD Service Coordinators assigned to the HCS, TxHmL, CFC, and GR programs. Will provide direct supervision, guidance and support, as well as initial and ongoing training/coaching to the team with clinical oversight from Manager of IDD.  Will work in conjunction with Manager of IDD to conduct audits; participate in interview process; assist with program planning; facilitate team meetings.  Will carry a reduced caseload for service coordination-assessing needs and desires, developing person-centered plans for services and supports, monitoring outcome-focused services, and providing crisis management and prevention.  Requires travel within the 7 counties.  Meets multiple deadlines and reporting requirements. May perform other work duties as assigned by Manager of IDD. Work is performed under the general supervision of the Manager of IDD with considerable latitude for the exercise of discretion and independent judgment.

ESSENTIAL FUNCTIONS and DUTIES:  Provides initial and ongoing training/coaching to the service coordinators per supervisory oversight.  Provides clinical support and guidance to team members (scheduled or as needed) per supervisory oversight.  Provides oversight and approval of team member's timesheets, leave requests, and schedules.  Provides monthly, individualized supervision to direct reports regarding job performance  Monitors productivity expectations of team and reports stats to Manager of IDD.  Performs team/staff evaluations and performance appraisals when due.  Facilitates team meetings as directed by Manager of IDD.  Participates in the interview/selection process of new employees for the Service Coordinator position.  Assists Service Coordinators in scheduling annuals, quarterlies and contacts to adhere to TAC and Gulf Bend Center policy.  Assists with program planning and development as delegated by Manager of IDD.  Conducts audits to ensure compliance according to HHSC contract, Medicaid Rule, and Center policy and reports all audit findings to Manager of IDD.  Provides service coordination to assigned caseload.  Responsible for assessing needs and desires, developing person-centered plans for services and supports, monitoring outcome-focused services, and providing crisis management and prevention.  Must maintain regular and reliable attendance.  Performance Appraisals for direct reports are 100% compliant (3 month, 6 month, and annual).  Relias trainings and any other training required for role is 100% compliant.  Employee supervisions (monthly and/or quarterly) are 100% compliant.  Approval of timesheets is 90% compliant.  Employee corrective actions are completed in a timely manner once the corrective action is identified.  Participates in the hiring process and termination process of department employees.  Complies with telework/remote agreement.  May perform other work duties as assigned by Manager of IDD.  Uses the trauma informed care approach when providing services and working with other.  Understands the Three E's of trauma-informed care.  Understands the Four R's of trauma-informed care.  Understands the six key principles of trauma-informed care.

KNOWLEDGE, SKILLS, ABILITIES, AND COMPETENCY:  Must have computer experience to include, Microsoft Word, Excel, and Outlook.  Must have good organizational skills to achieve multiple deadlines.  Must communicate effectively both orally and written.  Responsible for completion of all required documentation in an efficient and timely manner.  Strong decision making and organizational skills are required.  Must be self-directed with sound judgment under stressful situations.  Exhibits cooperative teamwork and models professionalism by appropriate dress, language, ethics and work habits.

Physical Requirements, Environmental Checklist, Work Site and Environmental Conditions:  Walking, operating office equipment, standing, operating motor vehicle, light lifting under 15 lbs, sitting, ability to see, identify colors, light carrying under 15 lbs, hearing with aid, straight pulling, ability to write/count/read/tell time, simple grasping, climbing stairs.  Travel by car 20% of time, outside, inside, working closely with others, working alone.

Other Requirements:  Current State of Texas Driver License or if you live in another state currently licensed in that state. Liability insurance required. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal care on Center business.  Good driving record as defined by Gulf Bend Center.  Computer experience to include: Microsoft Word, Excel, and Outlook.  Ability to type 40 wpm.  Must maintain regular and reliable attendance during business hours of Monday through Friday 8:00am to 5:00pm or agreed upon hours.
