POSITION TITLE: Service Coordinator Team Lead
WORKSITE LOCATION: Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904
STARTING PAY: $20.00/hr EEOC CODE: Professional FLSA STATUS: Non-exempt
SHIFT: Fulltime; Monday thru Friday, 8:00am - 5:00pm, with some variation to meet job requirements and customer needs
EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university OR an associate degree in a social, behavioral, human service, or health related field, including psychology social work, medicine, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice; OR a high school diploma or a certificate recognized by a state as equivalent of a high school diploma and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
ESSENTIAL FUNCTIONS and DUTIES:
1. Provides initial and ongoing training/coaching to the service coordinators and CSS per supervisory oversight.
2. Provides clinical support and guidance to team members (scheduled or as needed) by supervisory oversight.
3. Assist Service Coordinators in scheduling annuals, quarterlies and contacts to adhere to TAC and Gulf Bend Center policy.
4. Assists with program planning and development as delegated by Manager of IDD. Provides insight to determine successful caseload assignments.
5. Monitors and evaluates the productivity and quality of direct reports and conducts audits to ensure compliance in accordance with the HHSC contract, Medicaid Rule, and Center policy. Reports findings to Manager of IDD with a solution-oriented focus.
6. Responsible for providing service coordination to assigned caseload by assessing needs and desires, developing person-centered plans for services and supports, monitoring outcome-focused services, and providing crisis management and prevention.
7. Facilitate team meetings as directed by Manager of IDD.
8. Approval of team members' timesheets, leave requests, and schedules per policy is 100% compliant.
9. Completes performance appraisals, monthly supervisions, and corrective actions for direct reports in compliance with Center policy.
10. Participates in the interview/selection process of new employees for the positions supervised.
11. May perform other work duties as assigned by Manager of IDD.
12. Uses the trauma informed approach when providing services and working with others.

Preferred: BA/BS/RN/AAS: at least 1 year experience in providing mental health or intellectual disability services to adults, children or adolescents.
