      JOB PURPOSE: The Substance Use Intensive Case Manager (ICM) is responsible for intensive case management and rehabilitation services to people with a substance abuse disorder and/or co-occurring diagnosis of mental health enrolled in the Intensive Outpatient (IOP) Unit. The ICM is responsible for ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective. The position requires extensive knowledge of substance use disorders (SUD) and mental health (MH) disorders with working knowledge of treatment techniques that are effective for those with single and co-occurring disorders. The ICM will be joining a diverse, multi-disciplinary team and will also work with other staff at Gulf Bend Center for continuity of care and/or crisis services. Work involves planning, coordinating, and implementing substance use treatment activities, and coordinating with other programs and/or community organizations to ensure the maximum utilization of resources available for chemical dependency treatment and rehabilitation. Will carry a caseload and will also be responsible for providing a substance use skills group and individual services. ICM duties will also include and are not limited to providing continuity of care services, linking customers and families to all needed resources, and coordinating, monitoring, and updating services and customer goals, providing community outreach, and assisting in crisis prevention as applicable. Works under general supervision of the Specialty Clinical Services Manager with moderate latitude for the use of initiative and independent judgment.
